Aha! A Flash Gordon reference! It all makes sense now!
I find that the wording on Ming's Heart is not intuitive and doesn't follow other PoE wording. I had to read it a few times for it to register (pardon me I'm a lil slow).

It should have been:
20% of Physical Damage Added as Chaos Damage

Yes, it does. This is the same wording used on hatred and added fire damage.

No, it should not. That would be incorrect, as the damage is not "added" damage (which matters for added damage effectiveness). Hence why all similar things were renamed with the damage effectiveness fix to make it clear they did not count as "added" damage.

Yes the "added" fire damage gem needs renamed. I will try to make that happen for release.

40% of physical damage taken as lightning damage

so if I understand this one, instead of receiving 100 physical damage you would receive 60 physical and 40 lightning damage?

gain 20% of physical damage as extra chaos damage

and for this one if I was dealing 100 physical damage I would now deal 100 physical damage +20 chaos damage?

Add: in the wiki's unique history page, 2 entries are missing: 0.10.8c had introduced 'The Bringer of Rain', and with 0.11.1e we got 'The Blood Dance', 'Death's Oath', 'Auxium' and 'Soul Taker'. (And the 0.11.6 uniques are missing too here)
