Found by another user in reddit so most of the credit belongs to him. Just spreading the post for exposure. Apparently it's the Feral Ghoul in fallout 3

I feel like I've seen the art in the background of the zombie with the intestines spilling out before too. I want to say it's from an old Resident Evil game, RE1, RE2, or RE3 but could be from something else.

It would be pretty crazy if the art department had someone who was assigned to make art for that specific node and he had to wing it because he couldn't come up with something original within his time-frame.
