Not sure if this is the main reason but I do notice you have your cast when damage taken gem at level 19. That seems way too high since its something you want triggering quite often to mitigate damage you take.

Easiest/cheapest changes to hardest

-Your Dmg. on Full Life gem needs to go. Swap it out for Ruthless Blows which you have unlinked on 6th socket. Dmg on Full Life was basically dropped, DPS seems higher but it's not. It;s very spikey and unreliable. Ignore the tooltip!

-Your gem order is incorrect. Go to my gem section to order gems properly for max DPS. Gem order affects DPS.


Main Changes
-First, your belt is pretty awful for life. No reason not to have a 100+ life belt to reach 7k life. A 120 life dual res belt is actually pretty cheap. Come lv94 all life triples so a 120 life belt will net you over 300 life. 200 life Kaoms boots will grant you around 600 life. etc.

-Second, perfect kaoms boots can be a little rare, but for 5 chaos you can get another 90 life if you are patiently looking on poe.xyz for perfect kaom's boots. That's the difference between your 167 life and 200 life.

-Gloves with APS 16% from Master Crafting will help, scour the 14 to attributes via a master in case it will let you fit the APS mod that I believe Haku gives at lv6 or 7 for 8 alchs (will take 2-12 tries so stock up 20 alchs). 0.4% Leech will help too but APS is doing both a little each.

-Your rare ring has some sexy strength on it but the ring itself just needs to go. Find a ring and amulet combo where when both are combined you have enough-
intelligence
res capped
life both 75+
This is tricky because if you get rings that are perfectly covering your gaps in defenses then changing one gear means you might have to change some others as well. This is why dual striking is actually very hard to gear but it's the only way to min/max and is part of the process of being ready for survival

Just remember that rings and amulets can have 50+ intelligence on them. I used gear that would all work together which required calculating to the very stat point every single thing I needed so I knew before buying that it would work.

OPTIONAL- I accidentally removed it during the 3.0 editing, but a Belly of the Beast is NOT recommended as a 5L. You coukd consider trying to use Bringer of Rain til you can get it to 6L. You are sacrificing so much APS and Damage already for life by using Belly, and your axes are pretty decent so Bringer of Rain will tap into that. I'd wager a few chaos on a Bringer of Rain will help you wonders. It may seem like a downgrade but I'll bet you dollars to doughnuts the 7L of a Bringer will stop your dying with 2 Rumi's block flasks. Assuming you do not get a Bringer of Rain, helms can also provide tons of intelligence if you look. There are life helms that have 50 intelligence.

Just some suggestions! Nothing here is set in stone, just suggestions.

Very good eye! He's right bro. Get one that is lv1 up to a max of lv4. Rogue uses lv1 but I tend to use lv4

Make sure you have the Vitality Void cluster if you are going to Uber Trio. Cannot win without it! With it, you can take 16-18 stacks of corrupted blood with epic gear. Trio applies 8 stacks PER HIT at 1000 bleed dmg per second so be careful. My build has a harder time killing Uber Atziri than Shaper. Uber Trio should be fine though. Soup killed them a bunch of times with budget gear... just remember he was probably a better dual striker than me lol miss that guy!!!

Of everything that has been said so far, this is by FAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAR the most important, Cloaked in Savagery is essential, and should be taken FIRST, as it says in the guide. I would recommend you regret all your ascendancy points so far and get it asap. You cannot leech tank without it. Watch my videos and you'll see me going from 20% health to full in milliseconds and it should be apparent why it's needed.
