Hack 'n' Slash generally refers to games like Devil May Cry, Ninja Gaiden, God of War, Golden Axe. Where its more about relying on your skill to deal with the enemies, whether to pin-point-precision movements, flexible combos and juggling, or environment navigation.
I've definitely always heard Hack n' Slash refer to a style of RPG that is more about slaying enemies in dungeons than storytelling or puzzle solving.

Those games are definitely more about the action gameplay than story. Hence either focus on combos or difficulty. And you can't rely on gears to cover poor play.

My point is that I've always known "Hack n' Slash" to refer to RPGs that are about killing monsters even if that means you are relying on better gear and leveling up to do so over human dexterity, for the sake of doing it.
This is literally the first I've seen the term limited to non-RPG games like DMC or Ninja Gaiden.

They've been in that category for a long time. It also refers to ARPGs like Ys and Souls series (where you level up and get gear but that won't make up for your lack of skill). Heck, in most of the previous examples you also 'level' up with point distribution and get gear upgrades and aquisition.
The Hack n Slash is pretty much all about visceral combat over story or actual roleplaying.

It's a term that dates back to D&D table (AFAIK), so I argue that it has little to do with how much skill you need at the game and more to do with the context of the adventure, which is primarily killing monsters for the sake of it.
I'd definitely qualify PoE as a hack and slash RPG, even if it's mostly just point and click gameplay. This doesn't mean that games like Ys aren't hack and slash, it is just a very broad term that encompasses more of a context of adventure than how a game mechanically played.
