Kaom's dream: Triskeriaski and massive quantity of monsters(if you decide to rush through)

Daresso's dream: Stun lock in the pits if you don't have some good block / granite / some kind of phys mitigation.

Maps with -max res and extra cold/fire/lightning damage, monsters can have extra elemental mod too and kill you in one hit.

Devourers with dangerous mods like onslaught/extra damage/volatile.

Any volatile if you are melee and need to be close to the monster when it dies.

And of course, lag :P, most dangerous monster.

It is a bloodline pack mod.

When you kill a bloodline pack with the "bearers of flame/frost/storm" mod, each monster from the pack leaves a bomb of the specified element that explodes after some seconds, like 1-2 seconds.

Instant death if you don't see them.

Perhaps it wouldn't have gone the same way with the 5% HS and Splash buff.
Try rerolling HS next league you should do way better now.
