I found two things that significantly increased my FPS from 15 back to 60+ since the 3.1 patch.

1st- if your using Nvidia drivers / GPU
roll your drivers back to - https://www.geforce.com/drivers/results/116342

2nd - flush your TCP/IP
1. Right click on your Windows Start Menu, and select "Command Prompt (Admin)" or just search for Command Prompt and right click, run as administrator. (You should see a black box)
2. Now type in or copy and paste, "netsh int ip reset" Press Enter. Then do the same for "netsh winsock reset"
3. This should run through the TCP/IP reset.
Once it's complete, you'll need to restart the computer for the process to finish. Done!

Once I completed these two tasks my game was back to normal and FPS was no longer an issue.

I hope this helps.

Enjoy the game once again..

Thanks a lot finally fixed the problem.
Gpu was making noises even when the computer was idle which i found its totally weird but now its fine and my fps is back to normal i have to cap it to 60 to prevent my gpu from overheating.

Also you have to disable windows from updating graphic card drivers

THIS^^^

I am able to play at 4k now without dips to 30fps during large mobs. Lowest dip I have had is 51fps. So much smoother. GTX 1060 6gb and Core i5 4690k. Oh and I have Shadows and everything set to High as well.
Hey my Windows 10 cant handel this "old driver" sadly..
Edited: removed screenshot comparison as I did not realise dx versions impacted ingame graphics settings page.

Oh sorry i did not realise that about the sliding bar with the different resolutions modes.. can pretty much disregard the solution in my post in that case.

800x600 refers to window size it has no impact on quality, i always play window mode and i find i get the best fps when my window is at that size... I suggest you don't knock it before you try it, as clearly you are not having much luck with anything else.

Fair point, but its hard to take advice form someone that didn't know an option even existed ala the dynamic resolution bar.

Issues ongoing, even when attempting various resolutions.
nvm
