Are you running Dual GPU's? I used to have this same exact issue when turning SLI on, so I'm curious. I've since fixed the issue by adjusting my SLI profile to DX11 bits being turned on.

I should have posted my rig stats:

2nd gen i5 750 @ 2.67 GHz (built in 2010 -it's ageing I know this) P55 Express Chipset
16 gigs of memory
gforce gtx 560 driver version 364.72 (2nd gen PCI express)
win 7 Pro 64bit
2x WD HDs in RAID 0

I'm planning on building a new system in the next few months, but until that time, this rig will have to do. It's been very reliable overall and has worked fine with PoE until this latest patch. I can still play the game, but when I go into town or a H/O, or even some areas, the frame rate goes crazy.
