Hello,
I have some proposals:

critical
troublesome
monstrous
grotesque (I like this the most)
unsparing
distressful
painful

Hope I didn't repeat some of the others proposal. Sorry if I did.
From: About Path of Exile
In Path of Exile, the player has been exiled from their homeland to the corrupted and deeply hostile continent of Wraeclast. Alone, or with other exiles, they must develop their skills and discover powerful magical artifacts in order to survive the challenges of a cutthroat post-apocalyptic fantasy world.

From: The Caverns (last known world area)
As the hours drag on, my addled mind grows as exhausted as my sword arm. I begin to hear actual voices in the inky blackness, and a few times I catch a glimpse of an ethereal, fleeting figure, seeming to beckon to me. I start to hear words in the whispers, the voice sometimes female, sometimes male. "Come find me" it says, as I turn quickly towards the blackness. "I wait for you, lover". The words are confusing and maddening, what could they possibly mean? I have also begun to hear a quiet singing again, a different voice this time, drifting down from some high place. It is soft at first, barely audible, but it begins to rise as I draw nearer. The wordless melody is filled with palpable longing, with profound sorrow. I find my eyes welling with tears, despite all efforts to steel my heart. What miserable creature sings this song, so deep inside the earth? Ultimately, it mattes little. When I find the tunnel leading upwards to the voice, I ascend immediately.

From: Chris
“Currently, Path of Exile has three difficulty levels: Hard, Cruel and Ruthless. We're going to be adding a fourth difficulty level to the alpha realm soon, and need some ideas for its name.

The reason we want four difficulties is because three isn't enough to do what we want. No one will survive the fourth difficulty though, so the point is a little moot. It may be necessary for us to not put a specific name on the first difficulty (essentially "Normal", which we jokingly called "Hard" for now).”

Kudos for one of the most insightful posts that I've ever read on any forum!

General consensus / seems to be the most popular:
Outcast > Condemned > Banished > Exiled

From: boufbowl (another really nice take)
“The Path of: Exclusion, Exile, Exodus, Exoneration

Since this game's story is about the character itself and its growth, I thought that a good difficulty progression would reflect the character's path in life, starting from being "excluded" by his people, then "exiled", from there he takes an "exodus" to find himself and beat back the hoards of evil threatening the world, finally after he has done all of that and his name has become legend, the people "exonerate" him for his past misdeeds and allow him a place of honor back home.”

My thoughts:
I think banished and exiled are too similar to use as degrees or descriptions of difficulty.

Chris has said that, “no one will survive the fourth difficulty”. People are exonerated posthumously so boufbowl's idea for character progression could fit this scenario.

When I read The Caverns, I had the distinct impression that the character is about to meet (and mate?) with Merveil, losing his “human” life and / or sanity to her. (If that's the case, then I don't want to think about how the devs plan on handling the female characters' progression along these same lines.)

tl;dr
The player is exiled > is constantly challenged > survives against all odds > until his final ascension......

Exile > Confrontation > Defiance > Immortality (Starzon29 – Immortal)
Exile > Confrontation > Defiance > Obsession
Exile > Confrontation > Defiance > Insanity
Exile > Confrontation > Defiance > Eternity
Exile > Confrontation > Defiance > Destiny
Exile > Confrontation > Defiance > Ascension
They are looking a name for difficult level, not a title for hero.

My suggestion: chaos
Forgotten... because you soon will be.

Go for something new

Khaos = which is Chaos in Greek/Latin

There you go one more suggestion..xD

wrathful

unbreakable

iron willed

dreadful

fearless

defiant

One of these definitely fits.

And everyone pulls out their thesaurus.

Really, I think ness is on to something. Instead of naming the difficulties something generic you can use the opportunity to add a little more flavor to the game.

outcast -> forsaken -> banished -> exiled

I vote for this one.

Its not too cheesy and it sounds nice.
