filled Divine Vessel is missing

So with new league, I decided to continue my standard character for now
Because of usual changes in the atlas, I was forced to redo a bit of content, notably some pantheon powers, that were changed.

I'm not high end player, so I didn't had all of them unlocked this time and did two low tier maps (T3 Infested Valley and T4 Cemetery maps and got respective pantheon power vessels just fine)

but then I tried to do a T4 map "Lighthouse", after finishing map, the filled vessel was NOT in the map device (maps are current for this league)

After first try I was angry at myself as I thought I forgot to put vessel in a map device,while having only one Lighthouse map

I used bunch of Horizon Orbs to get second Lighthouse map and again divine vessel vanished.
So I did it third time
--- I checked if the vessel in in device before opening map
--- I checked if it is REALLY correct map acording to atlas
--- I checked if map has vessel modifier (+10% boss damage etc.)
--- I killed the map bosses (Redblade raider group)
--- I made sure that I have 0 monsters left in a map
and I edned with three spare portals, yet the vessel again vanished

as I said I'm not high end player, so I don't really mind loosing an item that I have limited use for, but it happened three times so I'm pretty certain it is a bug. and what bothers me is loosing time and othercurrencies that I have limited supply of (three divine vessels are half of the divine orb in vendor recipe)
I MAY have an idea where the issue is, but I am not sure
Map says I have to capture "Bin'aia, Crimson rain" but if I am not mistaken maps with redblade/mutewind/rothtooths does not always have the same composition.

I would really appreciate confirmation

Same issue for me on Lighthouse map.

I lost my divine vessel when using on lighthouse map as well. Seems like a likely bug

same issue and even if it fails, the divine vessel should never disappear.

same here this morning, t4 lighthouse, crucible league

Still not fixed, completed Lighthouse map in crucible league with divine vessel, it was consumed but wasn't given back on killing the map boss.

Just watched a friend do this on Lighthouse and he was also robbed of his filled Divine Vessel. Got rid of Stream of Consciousness and now he got his filled divine vessel properly.

Not sure if Stream effected it at all, as it explicitly says it won't, but he did get it after removing that node from his atlas.
