yep, same here
already ran Basilica a couple of times to see if it wasn't just my fault, however, there is still missing an organ or 2 after full clearing the map https://imgur.com/O8FmA5y |

I have experienced the same.
Zero mobs remain, missing one organ. |

I suspect the missing organ is because it's a two phase boss. The first phase doesn't drop an organ, but I think it's assigned an organ anyways, which leads to it always missing.

Same thing just happened to me, full clear and missing a final organ.
