hard, cruel, ruthless, ______ addition to current: hard, cruel, ruthless, brutal a twist on the current: harsh, cruel, ruthless, brutal a play on punishment: sentenced/condemned to: outcast, banishment, exile, death something straight forward: normal, hard, extreme, brutal you could always go with the ever classic doom difficulty settings: im too young to die, hey not too rough, hurt me plenty, ultra-violence, nightmare! or a mix of others: dont hurt me, bring it on, im ready for pain, blood bath! | |

just as an actual information: the 4th difficulty is named merciless and is going to be removed soon - so, there's no further need to suggest more than 3 "levels".
