Difficulty Level Naming

Yes good sir, I enjoy slaying mythical creatures.
I'll put Lethal and Horrific in the hat.
I wouldn't be here but I was out voted 3 to 1 by the other personalities.
I also like what ness and tpapp said about naming the difficulty to relate to the premise, like banished mode. Forgotten mode, Hopeless mode, Chaotic mode are some others.
Hard
Cruel
Ruthless

Inexorable
Sadistic
Wrathful
Petulant
Psychotic
Chaos
Pandemonium

It's really hard to describe past the mentality of ruthless in language that is short and catchy enough :P

Seems so. Most of these, I think, can be ruled out fairly easily. So, I'll just go through the list of what everyone's put down so far and asterisk the ones I think are at least somewhat fitting.

*Brutal
*Cutthroat
Relentless (a bit similar to "ruthless")
Merciless (also similar)
*Infernal (connotes hell, which may or may not be what the devs are shooting for)

Loathsome (these three I think convey disgust and not difficulty)
Abominable
Disgusting

Unfathomable ("unknown" difficulty?)

*Murderous

Suicidal (the others describe the difficulty, whereas this would describe the players attempting said difficulty)

*Wraeclast (of course I think my suggestion is decent)

Doomed (again, describes the players, not the difficulty)
Dratted ("mildly annoying difficulty"?)
*Forlorn (this is more "depressed" than "difficult-to-the-point-of-impossible", but does sound good and fit with the exile theme)

Self-seeking (again, doesn't describe difficulty)
Coveting / edacious (same as previous; also, "edacious" isn't a word)
Sateless (also not actually a word as yet)

*Outcast -> *Forsaken -> *Banished -> *Exiled

Indomitable (this does not adequately convey the fact that everything in the world is Out To Get You)
Bootless (regardless of the dictionary definition of this term, the literal reading of it will result in amusement rather than fear)
Nugatory (as previous)
Unwary (precise opposite of what we're going for)

*Inhuman

Vexatious (annoyance is not fear)
Amoral (As with "Indomitable", doesn't say They're Out To Get You)

*Lethal

*Horrific

Banished (already covered better elsewhere)

*Forgotten

Hopeless
Chaotic (confusing? Okay. What about hard? This word does not convey difficulty)
Inexorable (slowly-but-surely probably isn't how a difficulty should be played - or how it should kill you)

*Sadistic

*Wrathful

Petulant (kid throwing hissy fit does not a terrifying difficulty name make)

*Psychotic

Chaos (see chaotic)
Pandemonium (see chaos)

...and, after doing a few comments on these, one potential adjective popped out at me:

*Terrifying
I have wandered through insanity;
I have walked the spiral out.
Heard its twisted dreamed inanity
In a whisper, in a shout.
In the babbling cacophony
The refrains are all the same:
"[permutations of humanity]
are unworthy of the name!"
The theme is clearly the mental state of the creatures trying to kill you.

And I agree with most of your assessments of ideas put forth so far.
