Playing using a VPN is a great way to get your account information stolen or logged and expose your computer to unnecessary risks.

I also assume the latency is horrible when playing from a VPN.

Using a VPN to fix whatever changes they made in the last patch is NOT a reasonable fix.
So which new ports need to be open now?

a VPN isn't a great way yo get your account information stolen. Using an improper VPN perhaps.

Most VPN systems use a secure point to point tunnel which encrypts the data as it travels between endpoints. They do this for security and integrity of data. The only way your data would be stolen in this case is if the VPN gateway you're connected to is unsecured or owned by the hackers who want to steal your information.

With a properly secured VPN, it's actually more secure in many cases than just playing in the open, as in the open your information is much more vulnerable to packet sniffing/man-in-the-middle.

The reason the VPN is a recommended fix in many cases is not necessarily for connection, but to bypass caching. In this case where you can't connect to an instance, the VPN might work better because it bypasses some firewall and port blocking systems that are in place which could prevent connection otherwise.
How to find out which ports this game uses so I can unblock them?

I can login just fine, but everytime I try to start a game, it goes back to login screen saying failed to connect to instance.

Opening ports doesn't seem to fix the issue at all. I've tried opening every port as well as just disabling my firewall entirely to no avail.

Sometimes the game works and I'm able to play for about 25 minutes before getting an unexpected disconnection.
Why hasn't anyone from GGG addressed this :(

Where are you located, which Gateway are you playing on, do you use a wireless or wired connection, do you have anti-virus/firewall and if you do; have you added an exception for the game?

Can you also please try the following:
Download WinMTR from here: http://sourceforge.net/projects/winmtr/files/

Preferably while you're experiencing problems in game, get the server's IP address by opening the folder you installed PoE in and the "logs" subfolder, then open the "Client.txt" file, usually found here, Client.txt.

The file will probably be quite large, but scroll to the bottom and search for the last line containing "Connecting to instance server".

Example:
2012/03/27 10:33:25 3917590 50 [INFO Client 3768] Connecting to instance server at 127.0.0.1:12345

Instead of 127.0.0.1 it will have a different IP address.

Use the IP you find there in WinMTR in the "Host" box and click start.

Once the "Sent" column shows at least 50, click "Copy Text to clipboard", and paste the results into a reply (you can then just close WinMTR).

I play from India on the singapore server and ever since the patch .07 patch I have not been able to get past the character screen.I use a wireless connection. I followed the WinMTR steps as posted and this is the reply I've gotten :
