I always do them, but then I play self-found (in the regular league so I can group).

The Den generally gives a better belt than whatever I've got from A1.
A3 Fairgraves, admittedly, has never ever given me something usable. I have once seen a guildmate get an upgrade - to be fair, I think he was wearing a white coral ring until then.

Den gives a magic quicksilver as reward (it didnt in 2013 so maybe you arent aware), so i almost always do it.
In fact, even speedrunners do it for the qs flask. The power of 2 speedy drinks, over just 1, in acts were density is so low is not to be underestimated

Last time i did them was , when i was doing the achievements. Was something about doing all side quest .

Actually did Den a few leagues ago for the second Quicksilver Flask. I guess Medicine Chest on a second character might be faster, but I was too lazy to create a second char.

As to Fairgraves quest I only did it when I completed the acts for the first time - did all quests and explored every zone during my first playthrough.

same, do all the stuff I want because I run ssf and also because otherwise I'll forget something and miss a few skill points.

I check the quests before Katava and make sure I get all of them. The only one I think about not doing is the den right at the start, but I make myself not be lazy. People are way too obsessive about efficiency IMO.
