I have the exact same issue, i didn't play in ritual but i played in heist and i could run 100% deli maps fully juiced with scarabs etc. without issues, but this league i cant even run habringers/deli without reaching and spiking between 1/10/200 fps literally..
Tried out lowest/highest specs, no mtx/mtx on, no difference.

And I got 3080 with 32gb ram and i9 9900k...
Meh this really bothers me, not being able to play without dying and losing exp, and i thought "hey let's try HC next league" after dying 6 times to same map by lags screw HC.
Just chiming in to report I have the same problem (maybe GGG staff will notice this thread)

Specs: i5 8400, nvidia 3070 FE, game installed on a 1tb m2 SSD, 16GB ram

Same as OP, stuttering/spike issues when engaging with Ultimatum mechanics, lots of enemies spawning on screen/randomly after loading into a map. It happens only during the spawning of enemies, running around seems fine with nothing on the screen

Maps are unplayable as I die when a lot of things spawn and it freezes the game, 3 seconds later I regain control but already dead.
Specs: Intel(R) Core i7-7700 / Nvidia GeForce GTX 1070
Game has been set to the lowest possible settings with as many superfluous options turned off as it lets me.

As of a few days ago I would get lag spikes in my hideout when casting spells upwards of 500ms.

As of today:

When entering maps I have to wait for them to stabilize.
Any time I cast a spell the game becomes a slideshow (1000ms+ ping), if I try to give any inputs it just stops completely until I am dead because apparently the entities in the map are not beholden to the same limitations I am.
Any time anything out of the ordinary is present, harvest plot started, syndicate invades, there are more than 5 monsters on screen the game turns into a silde-show.
Its gone form me not being able to connect to the town or my hideout to simply not being able to play the game because trying to play it causes lag spikes that kill you.
This is of course not mentioning all the areas in the game loading their textures in splotches and skill effects being invisible or downright bizarre bugs where I fail objectives in syndicate somehow and enemies just non-stop using abilities like they aren't supposed to.

This doesn't apply to any other games I play. I have no idea what GGG did to their code in this update, but it has effectively ruined the game.

Same issue here still not fixed they blame us even some of us have the exact same knowlege or Networking Job as them haha
Same problems really.
Bit of potato pc but its never had these kind of problems with the game up untill a few weeks ago, with only the full spawn ultimatum stressing it but it was still playable.

But i dunno if it was the Nvidia patch or the POE patch (both came around the same time) but now the game is barely playable.
When i load into the map it takes about 30-60 seconds at times if not more to just render the textures of my buffs in the top left of the screen, they blur for a while.
My ice crash anim is just a big group of white squares for this time aswell, then when i get rolling the game stutters and sometimes just outright stalls when enemies come close enough for the game to start processing them.
I'll either end up fighting in a choppy mess or dead.
Ultimatums are a straight no go.

I thought it was lag at first but my net is fine and this is definitely massive frame drop, i have experienced lag enough to know it isn't that, I dunno what changed in that update but its like my game is trying to load and render everything at once everytime i go into a map (some of which i run 4/5 of the same map in a row doin dailies) and just borking itself.
I did think maybe it was my older hardware but since it's been fine and i am seeing people on their beast 3090s having similar problems i guess not.
Tried the cache tools, tried a reinstall, gonna try and roll my Nvidia driver back later but not holding much hope on that.

Edit - So ran the packcheck thing in the main folder, also uninstalled geforce experience (mostly because i was getting round to rolling back drivers) and so far it's been running alot smoother, my buffs dont blur, it hasn't stuttered much and i got through a few rounds of ultimatum, might work for others or not but so far it's made it alot more playable for me (for now at least it may still revert back) so just thought I'd share it.

New player here and i think i'm done. A shame, because i was (and still is) hooked.
On the other hand, i can't handle perfomance issues on a fast paced game.

I tried everything: Windows update rollback, Fresh installation, ISLC, command lines, reinstalling drivers... and the game is still barely playable. (Especially Ultimatums)

The true problem for me is that GGG seems not able to clearly communicate on the matter.

Thought I'd post a new reply instead of editing the one I had. Same issue with me and I swapped from DirectX to Vulcan, as well as included --waitforpreload in the launch params (Steam) and my stuttering/latency spikes disappeared. Give it a shot.
Sadly since the new patch, i also experience worse behavior.

AT first my RAM Usage spiked after 3min in a map - until my PC turned it self off to blue screen.

After trying all possible steps:
Verifing game files in steam
Deleting Cache
Checking drivers
Checking Windows
Checking different ingame settings
Checking my Router setting
Atleast my PC doesnt crash to blue screen anymore.

But since that, even on lowest setting, the game isnt fun/playable anymore 3/10/30 fps stuttering every mob Group.

And all the textures are streaming damn slow.

Specs:
i7- 8700 3,7 ghz
16gb ram
Win 10 pro latest updates
Geforce 1080ti
Game installed on ssd
Same problem Here.
My pc might not be that high (gtx 1060) but its more than enough to run the game smoothly yet still low fps and spikes when ever anything appear on screen epically trials.
I hope there would be a hotfix for it soon.
I have the same issue in Ultimatum Events, and there were a couple of small boss rooms that were actually considerably worse.

Sort of didnt matter too much levelling up, but with the high level maps, you cant freeze up like that, my XP bar is permanently at zero, sometimes I have to spin into the bosses direction, game freezes, hopefully I took some life off.. die.. repeat until boss dead (or portals run out).

I noticed in Diablo 3 there was a sort of similar issue which effected wizard builds (of which I was one), and there was a way to edit the sound file during bootup, which completely fixed it.. perhaps something similar here.. but it was a tricky fix because the game tries to fix it upon relogging.
