Actually, I have seen a lot of the fire mobs (late game) also having chaos damage. Odds are it was the Chaos damage that ripped you a new one.

I haven't done any maps yet but I've yet to find monsters that kill me on Act 3 Merciless with 80% fire resistance, or even damage me slightly.

I was just in a map where mobs did 99% dmg as cold, doesn't matter if you are maxed or not, when there's high ele dmg you'll get hurt.
