The game literally will not allow you to delete an mtx. If you really can't find it (I usually do something goofy like put it in my stash), try emailing support, they can find it for you (same might happen here too, just better not to fill up bug reports).

You you can't put items with mtx in guild stash. Or trade them. Or delete them. You have it somewhere. In your stash or on a character or in a crafting bench. Load up acquisition and search for it or email support and ask them to spend the time looking for it.

I know why I traveled without a helmet, then an accident happened two helmets (dupe glitch), one in guild, one for me, and a moment later a server crash, the server deleted one a helmet

Impossible to have one in the guild stash bcause you cannot ever put any mtx in the guild stash.

Install Acquisition... you can search for your mtx using that.
