As melee, it's almost always enough to have armor, endurance charges, and some leech.
You can skip a lot of these things, however, if you're skilled with granite flask use. Theorycrafter/Build Creator for PORTAL guild

I couldnt answer
since i never had problems with phys reflect ZiggyD is the Labyrinth of streamers, some like it, some dont, but GGG will make sure to push it down ur throat to make you like it
