What's wrong with my connection?

Your most recent WinMTR indicates the same issues posed by Brian, that something between your system and your modem/router is causing this issue. Without ruling out your cable or your modem/router there's not much more we can suggest here, other than verifying your system network activity with Resource Monitor, making sure your Antirivus, Firewall and Router are not filtering PoE and turning off any unnecessary programs while you play.

Small update.

I installed PoE on my parents computer alongside with WinMTR, and on this system, I am getting a different WinMTR result.

The system only got a Intel HD 2000 graphics, barely enough for playing, but even so, and at least for now, I am not experiencing any lag issues (network lag). I've made sure the firewalls and anti-virus are disabled. I've also ran WinMTR as admin.

I'm gonna do the following next:

- Play on my computer (the one who doesn't show 100% packet loss everywhere)
- Wait till I start lagging, then get a WinMTR log.
- While I'm experiencing this lag, I will head to my parents computer just to check if I also experience the same lag issues, and will also get a WinMTR log.

EDIT:

Running a second WinMTR on my parents computer and now I get results similar to mine.

This system's ethernet cable is 1.5m. My system's cable is not 5 meters long, but 15 meters - no idea how did I came with 5, sorry about that.

My modem got these options:

WAN Blocking - Enabled
Ipsec PassThrough - Enabled
PPTP PassThrough - Enabled
Remote Config Management - Disabled
UPnP Enable - Enabled
SIP ALG - Disabled

Shall I change any of that?

There's also a DMZ Host Option. I may try this at some other day.

Thanks for that Xarick. The latest test's still showing that loss to your router, so it could be helpful if you could test another router if one spare. If anything's off with their network settings, then usually just won't work at all rather than have problems like this.
Has any exile really been far even as decided to use even go want to do look more like?
Another update.

I had lag issues last night, and was even unable to login. So I went to my parents system and had exactly the same issue, unable to login. The two WinMTRs were very similar to what's been happening already, so no point posting them.

I'm starting to believe the issue is too specific. There seems to be an added delay to the downloading of game packets, they're put into some kind of "waiting queue", so they're not exactly lost, but the waiting can be for so long that WinMTR believes they're lost. They're just not delivered in a timely manner.

This is why I see strange things like the game suddenly entering super turbo speed mode, everything being catch-up after a very long lag spike, where I'm unable to "do anything". This "do anything" is, however "done", albeit blindly. That is, I can actually performs actions, they're received on the server immediately and handled fine on that end, but since the receiving of those actions on my end are put into some kind of "waiting queue", I don't see any immediate response in-game. Everybody else can see my actions, but I can't see my own actions.

This is what I believe it's happening. If there is a technical term about this issue, please tell me what it's called, because I'm sure gonna call my ISP and mention it. Calling my ISP is still my last resort. I have yet one last thing to try before that, which is DMZ Host.
