Constant disconnect on zone change

When playing, I often get disconnected when changing zone. Some time it get so bad the game barely unplayable, or really frustrating like earlier after killing Dominus, the game disconected me when trying to enter the portal and wouldn't let me log back in for 5 minutes. I even tried to configure my router port so has to not cause problem, here's an image of my configuration


And here's the result of running WinMTR for a few minutes, there's definitely some packet lost going on, but not quite sure what to do.

The packet loss on the single hop is not necessarily an issue.

Was this trace running/capturing when you had the disconnection problem? If not, can you please record another one that captures a disconnection so we can check the results in a more accurate scenario?
No, I didn't have the issue on that trace, I'll run it while playing and post another trace when I get the problem again.
I did a small mistake in my first post. I was running WinMTR on us server while playing on the european server.

Had to rerun WinMTR and connect to the US server. So, I got the problem while playing on and monitoring the good server this time.

Do you normally play on the US or European gateway?
US server, I tried to go on european server to see if it would fix the problem. I just forgot to set it back to US.
Does it only disconnect you while changing between zones?

I ask because I don't necessarily see anything in the trace to indicate a specific problem with disconnection just from zoning.
Yeah, the problem only occur when I change zone. I never got disconnected while going around a zone. I used this address US.login.pathofexile.com for WinMTR. I get desynch at time, but that's more a normal issue.
Do you have any antivirus or firewall software running on your PC?

If so, does it have the proper permissions and exclusions for PathOfExile.exe?

What about something like K9 Web Protection?

If it's only while changing zones, it seems like it is having trouble properly establishing a new connection to a new instance. This is almost always something filtering packets for the new instance. Usually firewall or antivirus, but not always.
I use Avast, I tried to disable it before, seemed to work for a while and then I got the problem coming back even if it was disabled. My router would be my only firewall, in my first post I posted my setup to open up the port used by PoE. I do not use any kind of software internet filter or parental control.
