This game has a basic survivability problem

dont go EV alone, build ES because of its scaling, get the glove to leech ES.
those monkeys designed the game this way.
I play a monk and did die occasionally until I tried out one thing.
Elemental expression, I use it only to perma debuff with blind and freeze everything. Basically everything is dead before they can touch you. Bosses by phase 2 are frozen as well.

Only thing, you need crit chance for this, over 50%.

This game has a problem with "range" on some unique enemies for sure, when others are tight, some of them are wide and can one shot from off screen.
Have you tried to cap your resistances?
Most of the time i die to an off-screen stuff is when I am not resi capped.
