balbala's barya in Cruel difficulty ACT 2 not dropping

I ascended twice already, once in act 2 and another in act 3. I'm now in act 2 cruel difficulty, and balbala's is not dropping the quest item to continue with the quest so I can ascend a third time. I have killed the boss four times by now, nothing. No NPCs, I can't speak to her in the previous act 2, nothing. I do not know what to do. I can't ascend a third time. The quest is stuck in "Search the traitor's passage". No progress from there on.

for me it dropped once as the green rarity on the ground and when i picked it up it turned grey like the other ones in my inventory like the one you can drop randomly but the quest is still stuck at "search the traitor's passage"

i think youre supposed to get it randomly. i still havent gotten a chaos trial token and i clear over 50 maps by now. i had gotten 1 barya token in the last 20+ hours (and failed). there is a node on the mapping skill tree that gives +40% token droprate in a specific biome.

I have the same problem.

Same issue, and more: the quest item version of the Ultimatum that's supposed to unlock the Trials of Chaos didn't drop from the Chimera boss in Act 3 Cruel, either. My guess is that the game thinks "oh, you've unlocked this already, there's no need to drop the quest item again", like how you don't get the quest to unlock the Salvaging Table in Act 1 Cruel. But instead, the rest of the quest gets orphaned.
(technically speaking, this bug will resolve itself when Acts 4-6 release and Cruel difficulty goes away, and you can just go back to Normal to play the two Trials, but that's a rather long time to have orphaned sidequests.)

yeah had that happen too and it looks like it is going to be bricked character(s) until there is a definite fix. it messes with my world map, I can't even get into the Act 3 trial area. (trial of chaos I believe) it bumps me down to Act 2 area.

I get map drops though so I guess it's not a complete waste..
just remembered that when the ultimatum(Quest Item) dropped I had a complete PoE2 CTD.

I didn't bother to look if it is my system at fault for that CTD, it only happened once, during a trial.
