I think I found temp solution for this.

I've changed options to:
- refresh rate from 120hz to 60hz
- render resolution 50%
- lightning: shadows (default)

So far no issues.

Edit: Also change network settings to "hybrid" in menus.

Wonder when this will be fixed because it's been more than two weeks and problem is still in the game.

I have 3 char this league and for me it happen only when I play 3rd one.
1. Necromancer
2. Pathfinder
3. Trickster

1st and 2nd doing more actions per minute than 3rd one but I don't get disconnected playing those two. On all chars LMB is binded to just Move and the 3rd one is using 5 skills (beside Move):
a) Caustic Arrow
b) Shrapnel Ballista
c) Dash
d) Clarity Aura
e) Malevolance Aura
Both 1st and 2nd have bigger skill arsenal and more active gameplay.
