It doesn't really look like you're handling it correctly here.

There is a togglable option called "Always Highlight", that is by default toggled with Z iirc. It looks like you have "Always Highlight" toggled off, because until you hold Alt to show the Legion Boots, the waypoint is not highlighted (showing the 'Waypoint' text).

With "Always Highlight" off, items on the ground are generally not displayed. Holding Alt only shows items that get through your loot filter. Kingmaker is often filtered out because of the item base, because Kingmaker, as a unique resulting from a vendor recipe, does not drop normally.

With "Always Highlight" on, items that get through your loot filter are displayed all the time. Holding Alt with "Always Highlight" on will display items on the ground that are hidden by your loot filter.
