Gifts of the Red Queen sextant not working properly sometimes?

Hello i used to run in the past and even now for farming specific bases a specific sextant:
"Your Maps contain 6 additional packs of Corrupted Vaal Monsters Your Maps have a 50% chance to contain Gifts of the Sacrificed per Sacrifice Fragment used Your Maps have a 50% chance to contain Gifts of the Red Queen per Mortal Fragment used" Sometimes while using two different Sacrifice Fragments which should give 100% for the strongbox to appear in maps, it does not appear. I've run only this league 200 maps with this sextant on and i've noted that on 35 of them the strongbox didn't appear. Corrupted vaal monsters appear always with the sextant on even on maps when the strongbox doesn't appear, this means there is only problem with the strongbox not having actuall 100% chance to appear with two different fragments or there is some bug that prevents it from appearing. This might not look like serious problem but it is making it harder to drop some of bases with rare mods and also wastes money spend on maps/other sextants paired with this one and also resources such as fragments and scarabs.
