Same problem.
Well same here since last league, huge lag spikes and crashes.
In other games like WoW or GW2 i do not have any problem with lag spikes etc.
I'm doing currently fresh reinstall and i found two guides and i hope it will help.

No more strange ping from european servers, no more strange grafics and most importantly no more insane latency spikes!

I´m happy now :)

Same to me.

Thanks that helps ! Looks like the texture streaming makes the latency broken.

Same issue here (Act IV) after patch to 3.14.0c I have around 300 - 400ms latency spikes. It's not playable !!!
Tried Frankfurt + Milano + London servers same problem.
I never have this problem before.
My latency to public Google DNS or pathofexile.com web server is 2ms.

Set networking mode to predictive and it's playable now. Latency spikes still but without huge lag in the game

I return networking mode back to Auto and seems it working. Here is my PingPlotter log + screenshot from PoE with latency graph. But I still have lag issue in Highgate. It's very laggy.

Ping to PoE Frankfurt server

I have the same problem, instances randomly becoming very laggy. Here is a WinMTR from today:

https://pastebin.com/peKzAJhC

It started right away at Ultimatum league launch, I didn't bother to look into it at first, since I could just create new instances during the Acts and be done with it. However, as i approached maps, some instances began to lag halfway through. It seemed fixed after I cleared the cached files as described in the general troubleshooting guide, but the problem resurfaced today just like it was...

Instances appear laggy, have lag spikes, but both things don't have to occur at the same time. Lag that 'looks' like lag (freeze followed by sped up action) happens with and without latency spikes. I suspect the packet loss shown in the WinMTR log is responsible for this, I'll probably have to play on predictive networking until a solution is found...
