I had a rigwald's curse drop in a same way from ritual loot explosion.

I also got 1 Currupted, Unidentified Blightwell Talisman, it was in a Tier 1 Map, from a Blight Encounter (not sure if it was from a chest or the general loot at the end)

I just got one too. I think it was just a random drop in a t16 map.

I just got the unidentified version from A9 Blood Aqueducts Vaal side area as well :)

I got the 'Black Maw Talisman' unidentified and corrupted from the 3rd Ascendancy Trial. Trash mob drop. I'm pretty sure it is supposed to be 'Night's Hold'.
