I feel like ur defenses are lacking. U should at least try to get Fortify with shield charge otherwise u get oneshot eventually once ur block fails.

I'm running Ascendant myself first with Reap in mind, but swapped to Exsanguinate bcz of the lack of clearspeed on Reap.
Using max block glancing blow, 60/40 dodge and high evasion + fortify as low life with petrified blood. My gear is still bad and Evasion doesn't kick in yet, but it feels pretty good. Only thing I really need to figure out is how to get life leech going without the need of the Life Leech Support. (maybe Atziri belt)

Should the defenses still not be enough I might go for a %life on block shield aswell.

Forgot one thing:

Cast when dmg taken can be very unreliable if you are blocking all the time.
You might want to try Arcanist Brand instead. You have to cast it and put in on the ground but once a monster walks onto it and it is attached to the monster Arcanist Brand will keep casting all linked spells for its entire duration.

Not sure how well it would work for this build, but might be worth a try

Exsanguinate for packs reap for bosses, simple as.
