If you haven't already, could you please try uninstalling your graphics drivers and performing a fresh install of them, in case a driver corruption is causing the problems?

If you have done this, would you mind providing a fresh DxDiag report for us to check over? I'm sorry for the hassle, but if your system configuration has had any changes this may be reflected in your DxDiag report which may help us narrow down where the problems are occurring, if the issues are persisting.
