Yeah its like PC is getting a hick-up.

This is happening only in PoE or in all games You play?

So cause is not Your rig but soft. I have same issue. Other games works fine but PoE get those "hickups". Ppl reporting about this happening on ANY rig. What You can do is minimise this thing but... well.

How much RAM do you have and how fast is it?
Your CPU is fine. If anything toss a few bucks into a new gpu.
Most of the issues you're having sound like latency problems other than your cpu being at 100%. I have the i7 7700k and run capped at 140fps on my 2k 144hz monitor. I was even running on my 4k monitor at a constant 60fps.
Although I have 32gb RAM and the GTX1080Ti.

I have 16GB of DDR4. Am I able to just upgrade my GPU with my current setup?
What do your F1 stats look like during those pauses?

If you see actual fps drops in the stats, then yeah, go with one of the upgrades people are recommending. If you don't see fps drops or latency in the stats when you stutter, the stuttering/pauses have unknown cause since 3.10 dropped and the only way to get rid of them is to completely replace your entire machine with a carbon copy of a machine that isn't getting these stutters (including drivers and all other software). In other words, it's impossible to diagnose.

I've been getting the same weird stutters (sometimes my inputs are dropped for up to 3 seconds, horrifying even in softcore), and I have a beefier setup than you do. I even have dynamic resolution turned on, I never get graphical drops (well I do in ultra-dense delirium double beyond blight situations, but the takeaway there is that the dynamic resolution is working and not causing stutters), the f1 stats never show a problem, it's also not strongly correlated with a ton of action happening on screen (it's just, the only time it actually bothers me is when there's a ton of action on screen), it happens in my hideout at about the same rate, for no reason at all. It's not a hardware problem, it's definitely a software problem, that I guess is only triggered by some specific drivers that GGG isn't working well with.

Sorry it's happening to a HC player.

Yes you can buy a new GPU. I was fortunate to get the 1080Ti at release for the low low price of about $800. And they are still about that price if not more. Very good card. But if you don't wanna spend that much you can get the 1060 (perfect if all you're doing is 1080p resolution.)
I don't know much about the RTX and the 2XXX series cards though. Maybe someone who does can chime in.

Game pauses up to 5 sec long does not sound like a hardware issue, especially if there are times when the game runs fine. You might check your antivirus to see if it is flagging PoE as harmful.

It could be any number of things so it might be useful to check the event viewer in case you have some other weird issues like graphics or NIC drivers crashing or something.

I've also heard that being on Windows 7 is pretty bad for playing PoE. Not sure that is relevant for you.

That all being said just buying a new computer will fix your issues (assuming they are not internet connection issues), so considering the age or your hardware, if you have the money it might just be worth it. Getting new toys is always fun :D
