Unbearable Whispers V Prophecy not Triggering

The Unbearable Whispers V prophecy is not triggering upon completing the eternal labyrinth. In the past this prophecy has always been a guaranteed trigger. After 2 lab runs (both eternal) the prophecy still refuses to trigger. For good measure I tried to do other versions of the labyrinth and those failed as well.
