In a general sense, kill first loot after is the way to go. But picking up things while VMS is up doesn't really effect its uptime since the soul cost is really low, especially with 3x30% Sacrificial harvest. It's just a thing that you'll have to get used to.

I'd drop HoI, and don't see the usefulness in it. I doesn't provide more AoE, since the build's AoE is focused around spreading decent ignites with Beacon of Ruin. And the range of Beacon of Ruin, with all the global AoE scaling, is a lot larger than the AoE, with an AoE gem, on HoI. It's basically a dead gem as far as I can tell.
Since this build's AoE is based around spreading ignites, specing into more damage and using the Anger aura (Anger is basically a 20% damage boost for this build) helps a lot with coverage. If you doesn't have good ignites, you'll just end up pinballing around and backtracking into packs that should've been cleared if there were more damage to scale the ignites off.
At last! You replied. :) Jokes aside, I've listened to some of your advice and I can confirm that insanity gloves are day and night. The difference is tremendous. Furthermore, I've upgraded boots and they make everything just a tad smother, when I just need to do a tiny reposition or whatever. Furthermore, a silver flask does wonders. I'm getting used to the speed of the build and the fact that picking loot up is not a priority anymore. Unless it's something really worthy, I'm generally just continuing and depending on the amount of loot there is, I either skip it entirely or go back for it. I'm looking into getting anger as I've realized that the ignites are what does the job for packs, but I'm left wondering, how to deal with certain bosses. The problem I encounter are bosses with multiple phases, whose fights extend over the duration of VMS. Generally, I save a charge of VMS for the boss and then down him within a second or two by channeling a flameblast, but when the boss becomes invulnerable due to its phases, VMS runs out and I have to use flameblast. Sometimes it becomes really inconvenient and I'm at a loss. How do you deal with such situations and is there anything to do? Anyway, here's my gear again with those aforementioned upgrades.

I should probably write a section about this in the thread. But for maps like Palace or T16 Minotaur (which is the only t16 map this build could do if you got the gear requirement for it) you'll need a second VMS (maybe a third for the Minotaur). Vaal charges gets stored in the instance, if you put a charged Vaal gem in your inventory it will keep the charges for that instance. So just charge up a second VMS end use it when you need to!

Technically you could wear Kaom's roots and shackles of the wretched and curse youself with temp chains and not be affected by the movement speed penalty but you lose movement speed on boots so it sort of doesn't really help that much. This is a bad idea! Just saying technically if you were going to try to do that, it is possible lol.
