As stated in the Weekend of Races announcement thread ( https://www.pathofexile.com/forum/view-thread/1759833)
"
By reaching level 10 in any race you will be awarded a Carnage Mystery Box! This applies only once per account. Multiple level 10 characters will not grant multiple mystery boxes.
The prizes will be awarded after the weekend of races has concluded. We'll be issuing these manually during the work-day.

There was an oversight with the way the mystery boxes were awarded, we are currently working on awarding the remaining boxes to players who have not received them yet. Sorry for the delay.

Well, all boxes are supposed to be awareded but I haven't received any box yet. Had for example a lev 11 in the winterheart race. Character still exists in my account....
