I have been running something very similar to this for the last couple weeks and it is awesome.
I took less hp and resist nodes though and opted for more damage. Everything is either chilled or frozen for the entire fight and the added meatiness of your minions makes this a very solid glass cannon build with a huge amount of indirect survivability. I like to call it the 'Necromantic Frostotem Templar'. It is very important to combine the words 'frost' and 'totem' or the whole build falls apart.
