Oh yea, I should clarify. I was in Warbands, if that makes any difference. I can't imagine bugs like this are limited by league but you never know.

I'll bump this because it's a serious issue, and you have to kill all 3 guardians again + Malachai's first form again if it happens.

Same, I never got to fight a heart. He went perm invulnerable at about 75% health. No shield and it sounded like I was doing damage but hp never moved again. He finally killed me with his spawn explosion while I was trying to find something else to attack. First weekend of 2.0 a bust for me, rawr! Tempest

Got the same Bug, had it once after the first heart and once none of them activated at all.
Does it work at some point or is it bugged no matter what you do because I killed the bosses beforehand twice now and I really don't want to do it anymore if if wont work anyways... |

Same problem here. Stuck at 5-10% hp and the 3rd heart won't activate.

Failed 3 times then a friend invited me while I just stood in the room, it worked then, don't know what did it. I'm using Magma orb, maybe he bugs if you use that? I didn't cast it the time it worked, maybe it was him opening the instance. No Idea.
