One possibility for the false-flag is if you're using software that is implemented by injecting a DLL within the target process. A good example is 'proxification' software (e.g, proxifier) of which is often implemented as a psuedo-LSP that is injected automatically into every process (thus, even though you may not have configured such software for PoE, it will inject itself regardless)
The registry value AppInitDlls contains a list of such automatically injected DLLs in the key: HKEY_LOCAL_MACHINE\Software\Microsoft\Windows NT\CurrentVersion\Windows Also, debuggers (or software implemented as a userland debugger) will also set off red flags.
