The prefix and suffix don't determine which modifiers are given, they ARE the modifiers on an item.

A rare item can have 3 Prefixes and 3 Suffixes.

Go to http://www.exaltingpoe.com/ and look for yourself.
Choose an item type, choose the item and click next.
Then you can choose 6 modifiers.
You can determine all 6 or none, it doesn't matter.
Click finish to see a table of all possible Prefixes and Suffixes for that item and also how many you already used on the item.

Also, like everyone said before. The name of a rare item doesn't mean anything.

But from what i have seen, lots of item have similar name but theirs mods are different.

And. if an item rolled a few low value mod( i mean like 16hp etc) , average ppl wont put an exa on the item.
