In-Game latency vs WinMTR vs Game Lag?

additional info... happened again.. this time the pingplotter showed similar packet loss but on a different networklayer.com server... po1.fcr01 this time. got as high as 70%.. Interestingly, this time the flatlines where shorter - i.e. only 3-5 seconds vs the previous 10-20sec... but still noticeable.
