Another round of lag spikes and DC

I posted here a while back regarding lags and disconnects from EU / American servers, I was pointed out to that my problems resides on my end since I had packet loss to my router, since then I have fixed that problem and am now having even worst lags than before.

I have attached a WinMTR printscreen:

All the redacted parts are my router and ISP hops, as far as I can see there aren't any packet losses, and the ping times seem OK to me (I'm not used to extremely low pings). I have tried connecting directly to my modem bypassing the router only to get the same result. I am connected via a wired connection and have tested with my firewall disabled. I am not experiencing any lags in normal internet use or even downloading processes (mind you they are not running while I'm testing POE just to be clear).

In game in EU / American servers I experience either times of constant high lags, or just big spikes making POE unplayable, I have tested this several times to ensure this wasn't some peak time. Any help would be appreciated.

not sure if this will help your problems, but check to see if your video drivers are updated and next enable vsync in the game opitions. I had huge lag problems and when i did the following above, i don't have as much lag more playable now. let me know if this makes a differents in the lag.

Now with your disconnecting im not sure how to help you with that issue, i haven't deced yet
Hmm well I have checked those per your advice but to no avail, though I'm not sure it's related, it's not manifesting as any FPS drops or anything just lags and hangups.
Was this trace capturing at the time you experienced the latency and disconnection?

The trace isn't showing signs of a problem (that doesn't mean there still isn't I've). The trace is only measuring ping times and it's possible that the problem might be a deeper form of inspection which doesn't show up on ICMP packets.
Well since I'm experience that problem all the time, in this case I was running it while trying to play, yes.
To bad wish it did help you out, but im going to let Drakier help you, hes the one that helped me fix my lag problems, pretty sure he will figuer it out for you,
Just to make sure, you've disabled all antivirus/firewall/security software and hardware?

Do you have any way of testing the game on a different PC on the same network, or trying your PC on a different "known good" network (like a friend's house)?

Have you attempted to use a VPN to play? I know the VPN is going to have more latency, but does it remove the disconnection issues?

Do you only get disconnected when changing zones, or does oy happen randomly while in a zone? Does the trace you posted include a disconnection, or just latency?
The paste includes only latency sadly and I'm experiencing this while playing, more noticeable while fighting mobs than just standing in one spot. Disconnects are aswell usually while fighting, I get hung up at like 2000k ms for like 30 seconds followed by a disconnect or just an instant disconnect in the middle of nowhere.

I have disabled any software / hardware antivirus or firewall for the duration.

It will be a bit hard for me to test via a VPN but I will try to though I have never had any serious network issues with this connection. It will also be a problem testing it on another computer since no other computer I have can run POE but I will try it on a friends network. Trying to rule out this specific computer I can successfully play other games without these spikes.

I'd be interested in a trace containing a disconnect.

When you experience this latency, does your screen freeze up, or can you freely move about the map but no combat or actions work?

It's starting to sound more like a local machine issue and not something network related.

I've seen other complaints about similar issues where it turns out their video drivers were being forcefully reset by windows and it caused temporary lockups followed by disconnection (due to heartbeat timeout).
I can not play this game because of freezes and lags. (11/11/13 - 19:20 Braziliam Time).
