The vid you linked is from two years ago, and the situation you mentioned... He was facetanking what I believe was Torture Chamber boss' totems. Doing that is just incredibly stupid, I have no idea why he was doing that. Rest of the deaths were player mistakes, apart from one desync and desync's already been (mostly) fixed.

The topic you linked has also been dealt with by GGG; leapers have an instance-wide cooldown on their skill, so getting jumped on by a whole pack doesn't happen anymore. Still, Tora missions are a death trap and you should watch out whenever you do those.
