Failure to connect to instance

When I get this issue I end up getting the unable to connect to patching server to issue. I have read all the ways to possibly fix these issues. The only thing that solves it is when I unplug my router and plug it back in. Which obviously means it has to be on my side somewhere. I have done the port forwarding(of a retard amount of ports you guys asked to be done personally...half of the total amount...seriously, nothing more specific?)... And when this happens I tend to get connection issues to Path of Exiles site as well.

I have Cox, I live in the Phoenix area of Arizona. All my other connections work fine when this happens. It has never happened before until this year(2015). Then again I didn't play the Torment/Bloodline league so it might of technically started when that league started.

So if anyone has any solutions that I haven't came across please let me know.

Here's what I have done so far:
-Forward the ports that were asked for
-Opened up the firewall the the path exe
-I tried switch gateways then back to solve the issue
....I connect to EU easily but I am not gonna play with 200+ ping obviously by staying there
-I did the shortcut change to change the port on startup so I skip the patching server

What's the patch server ip/address? Tried looking it up but don't want to dig around the multiple page threads that it is probably located in.
Although I think I kind of solved the issue. I originally just allowed the path of exile program in the windows firewall. I now went in and selected each of the exes(all 4, granted I know 2 of them shouldn't be needed). And now I don't get the permanent disconnects but I still get the 'failure to connect to instance' but I can log right back in. Maybe with 30 extra seconds on load time but that's it. I also have to completely close the program to for it to connect fine.

Still leave the patching server ip/address just so I can do it when/if the problem happens again.

Edit: And now when I reply I am getting the problem again(to the point of needing to restart my router). If it does end up being the way my connection reaches POEs server, is there anything I can do about that? Obviously I can't fix the DNS server it'd be happening on but can I alter the path my connection takes to get there?

To use WinMTR:

Step 1 - Download/Run WinMTR:

http://sourceforge.net/projects/winmtr/ (get 0.92)

Step 2 - Host field:

If you use the Australian gateway, please enter 'AU.login.pathofexile.com' or '119.82.146.48'
If you use the Europe gateway, please enter 'EU.login.pathofexile.com' or '37.58.67.210'
If you use the Singapore gateway, please enter 'SG.login.pathofexile.com' or '216.12.203.91'

If you are diagnosing your connection to the patching server, please enter 'patchcdn.pathofexile.com' into the host field and click Start.

I wasn't using 0.9.2 before. The download now button on that page is 0.8 ... So 0.9.2 seems to have given more accurate results(and the hosts names finally)...thought that was weird that they didn't show yet they would show(while 90% of them) when I did a tracert in command prompt

Yeah, sorry dude - that is some nasty packetloss from Hop 2 onwards. The only way to get that fixed would be to bug your ISP.

The good news is, with the lockstep system being introduced later this year, packetloss shouldn't have as much of an impact as it does with the current client prediction model - it will be laggy and jerky, but hopefully will not result in as many log outs.
