Game Crashing since 1.2

It's been happening since the release of forsaken masters. I log in, and somewhere between a few minutes to an hour, the game dies. It will either just freeze and then a "PathOfExile has stopped working" error message comes up, or the screen will go black a second, come back up, freeze and then after a few minutes of waiting, the "PathOfExile has stopped working" will come up.

Find the lates minidumps and anlyse them with http://www.nirsoft.net/utils/blue_screen_view.html and post what files cause the error

ntoskrnl.exe seems to be a recurring problem with system crashes.

Perform a clean install of your video card drivers deleting all NVIDIA components PHYSX, Audio Drivers etc. In NVIDIA install options check "celan install" and Install only PhysX and video driver and see will the problem occure again.

I've already re-installed the drivers multiple times which have not resolved the issue. I've reformatted and re-installed everything twice now.

If you're getting ntoskrl crashes even after formatting and re-installing everything, then there is something else that is wrong.

Those crashes aren't normal. This leads me to believe that you have a hardware failure somewhere.
