Died during Mercy Mission

A friend in my party died while we were killing the boss for Mercy Mission, we killed the boss while he was running back to us and the Medicine Chest didn't spawn for him. This may be the way it's meant to work, but if not here's a bug report.

If he was not in the instance during the time of the bosses death, then the quest item will not drop for him.
Then what he have to do now to get the Medicine Chest?
I'd venture a guess he needs to kill Hailrake himself in another instance to have the quest completed.
This signals the end of a post. I apologise to everyone who had to suffer through it.
How do you do that?

That's a pretty terrible mechanic in an otherwise solid game. And the flow of creating a new instance isn't exactly obvious to new players which could easily lead to frustration.

You should consider allowing dead players to get quest items when their pary-members complete a quest or make it MUCH more obvious on how to recreate the instance when this happens so they don't spend too much time falling behind/holding up their party. (But I'd go for the first option, not much fun having to repeat a quest while your mates progress without you.)

Dead players can get quest items. A player which has revived, leaving the instance, and not come back, can not. The game can't tell whether they intend to come back to the instance at all in that case. If you stay dead on the ground because the boss is about to die, you're still in the instance and the item will drop/quests states be granted.

Thanks for your reply - that makes sense - is this communicated to the player or am I just being impatient and dense and missed it in my haste to get back into the action?

Perhaps they could implement a way for the game to determine if the quest state doesn't match up with the current state of the Instance the player currently resides in and display a "Reset Instance" button somewhere (Quest Journal [U] maybe?).

Obviously this approach wouldn't handle non-quest related situations.
