Build Feedback 2H Sweep+Glacier

Please judge my build, currently on lvl 51, cruel Act III and having severe problems with survivability.





I am not sure if it is build or my gear.


I have like 1400HP, 1700 Armour, 500 dps on Sweep, 2000 dps on Glacial Hammer. Was forced to use Warlords Mark as a support for better survivability.
