Nonstop casting spells for zero mana.
Edit: As people have pointed out, Righteous Fire is degeneration damage and pointed to "Blood Magic" not being considered as damage taken.

No, it does not. ES cares that you lost life (or es), not that you did so because of takign damage specifically. If blood magic was taking damage, then shock would increase the cost of everything by 40%.
Paying life with blood magic is not taking damage and will not trigger Cast on Damage Taken.
The reason I ask is that RF is increased by burning damage passives (so I imagine it is thus considered "damage").

I'm not sure what I have just read...
EDIT: A ok I get it, but I think DoT effects aren't counted as taken damage.

I'm pretty sure that righteous fire is also not considered as taking damage.

You can test this in-game right now by seeing if RF degeneration increases while shocked.

Righteous Fire does indeed deal damage, but the Cast on Damage Taken support is specifically tracking damage from hits, and thus not damage-over-time, similar to molten shell.

If I understand correctly, then reflected damage would not count, but Lightning Thorns would?

They'd both count, unless of course Reflect doesn't count as a "hit", in which case neither would count.
But Blood Rage wouldn't count.

Lightning thorns is reflected damage. Which does cause a hit (although for quite a while it did not), and thus will count.

Reflected damage makes molten shell go off so ya I would think reflected damage would make CoDT proc.
