Quest - zone progress stuck

The Well of Souls instance does not trigger the Black Cathedral portal. All dialogue options with the Lurking Creature are exhausted and the Kulemak topic never activates, even after relog and new instance.

Do you have Kulemak's Invitation in your inventory? That will spawn the entrance to the Black Cathedral.
