Weathered in light graft vanished from my action bar….

Was working fine then the command action bar skill disappeared and I’ve tried everything I could think of to get it back on.
Anyone have any clue on what to do?
I’ve tried switching grafts, I’ve reset the game, I’ve did /bug / debug I can’t figure out how to even post the screen shot.
