Omen of amelioration doesn't proc

hi,
as the title said, my omen of amelioration that i have in my inventory and who is activated ... didn't proc when i died on a delirium forge map. Have you died on this map before or have other omens triggered?

no i didn't die before (i do map with 1 portal only so if i die once, the map is lost)
