Curses Not Applying In Maps or Trials?

My friend is playing a curse bot with Blasphemy, she has Lich and doedre passives so should be able to apply multiple curses from blasphemy, but somehow no curses work in trials of chaos or only despair applied in maps. We went to a random area in campaign and the curses applied properly.

I'm really confused if this is a bug or a weird scaling issue with multiplayer. She did mention the curses seem to work fine with another player.

The curse gem is too low level. It needs to be high enough to apply to higher leveled enemies. Check the gem.

Oh I didn't know curses have a minimal level threshold on enemies. I see the numbers go up per level but didn't think lower levels straight up didn't apply.
