Every boss fails to transition phases

It seems like every map boss if they have a phase transition will get stuck and be unable to transition to the next phase. AI breaks. Not Just black crow; it has happened with every iteration of map boss across seven characters which are doing different things when it happens. Me and Manoki, The Chosen currently just hanging out lookin at each other as i type this up. Does this always happen? no, i've fought him three time this morning successfully however it happens at least once a session if not more.
